Authorization and Release
In consideration of Canada Quality Meds filling the prescription for my medication, I confirm the following information and provide the following releases:
My Medication
1. I will not use my medication except as prescribed;
2. I will use this medication for personal use only; and
3. I will not make my medication available for resale.
About me
1. I am the age of majority in the state where I live;
2. I am entitled to make my own medical decisions under the law of that state;
3. The medication I have ordered was prescribed by a licensed doctor of that state; and
4. In obtaining the prescription for my medication, I have not broken any laws of that state.
Delivery of my medication
I name Canada Quality Meds as my attorney for the limited purpose of signing any documents as required to expedite the delivery of my ordered medications.
The Releases
1. I release and discharge Canada Quality Meds and its directors, officers, agents and
employees from any and all liability, claims, actions or cause of action arising out of the
use or application of my medication, including but not limited to undesired side effects,
and this release discharge extends for the benefit and protection of any doctor who assists
Canada Quality Meds in filling the prescription for my medication; and
2. I release and discharge Canada Quality Meds and its directors, officers, agents and
employees from any and all liability, claims, actions or cause of action with respect to
errors or omissions by the carrier responsible for delivering my medication to me.
